When you supply your personal details to Therapy and Fitness (includive of Simply Osteopathy) it is stored and processed in line with the Data Protection Act 2018, which includes the General Date Protection act

The   Personal Data We Process and What We Do With It

We record and use the following categories of  personal data:

Name, address, telephone numbers, email address, date  of birth, health information including medical history, diagnosis and  treatment data.  This is inclusive of illness, medication and accidents you may have had previously. We need to collect this imformation becuase without it we are unable to do our job effectively and safely

Our lawful basis of processing this data is one of  contract, legitimate interest and, for the health information, the provision  of health-related services as a chiropractic clinic. In addition, we will only examine or treat  you with your explicit consent.

Sharing   Your Personal Data

We will never share your data with anyone who does not need access without your explicit consent. If you have any concerns about the sharing of your data have the right to complain to 'The Data Controller". Here are the details :

Retaining   Your Personal Data

Whilst you are receiving treatment from our clinic  we will continue to store and use your personal data. Should you discontinue treatment we will be  required  by law to retain your personal data for a minimum of 8 years. In the case of children, we have to keep  their files for a period of 7 years after their 18th birthday.After thid period you may ask us to delete your records if you wish. 

Data Storage

Any records since August 2018 are stored online. Any records from attendind appointment previously are stored in a lcoked filling cabinet. and keep for up to 8 years.  Our computers storing your information are password protected and the passwords are changed regulary. The information is the property of Therapy and Fitness and will not be given to any third parties without your consent. 

Your Rights

As we process your personal data, you have certain  rights. These are a right of access, a   right of rectification, a right of erasure and a right to restrict  processing.

· You may request   a copy of your data at any time. All   such requests must be made in writing or by email to us using the contact details shown above. 

Please provide the following information:

Name, address, telephone no, email address, details   of the information you require.

· If you believe   any of the personal data we hold on you is inaccurate or incomplete, please   contact us directly and any necessary corrections to your data will be made   without undue delay.

· Where you have   provided explicit consent for us to use your data you have a right to   withdraw this consent at any time.

Data Breaches

Should your personal data that we control be lost,   stolen or otherwise breached, where this constitutes a high risk to your   rights and freedoms, we will contact you without delay. We will give you the contact details of the   person at the clinic who is dealing with the breach, explain to you the   nature of the breach and the steps we are taking to deal with it.

Automated Decision Making and Profiling

We do not use any system which uses automated   decision making or profiling in respect of your personal data.
